Overview
In this short course designed especially for WSH professionals and managers who are tasked to carry out accident reporting and investigation, the following topics will be covered:
Objective
Participants will be introduced to an Accident Investigation tool known as Systematic Cause Analysis Technique (SCAT)®. SCAT is a systematic approach of accident investigation which will help users identify areas for corrective action by understanding the immediate and root causes of how accident happened.

Mr. Goh Chye Guan

Chye Guan has close to 40 years of workplace safety & health (WSH) experience from both the public and private sectors. He is the founder of SMER Pte Ltd that specialises in WSH culture, planning and leadership services. In his previous employment, Chye Guan travelled widely and has worked with the Ministry of Manpower, WSH Council, Det Norske Veritas (DNV), Singapore Construction Safety and Consultancy (SC2), and Integrated Risk Management (IRM).
Chye Guan last held public sector appointments were the Director (Industry Capability Building) and Director (Industry Sensing and Engagement) of the WSH Council, Ministry of Manpower. In WSH Council, he conceptualised and developed bizSAFE. In DNV, Chye Guan has extensive use of the International Safety Rating System ISRS) and the International Contractor Rating System.
Chye Guan has hands-on experience in accident investigation during his employment in the Ministry of Manpower and DNV.

An e-Certificate of Attendance will be issued to participant who complete the course with 100% attendance.
Replacement/Postponement
A notice of replacement / postponement together with the full particulars of the new participant (for replacement) must be given to us AT LEAST 3 working days before course commencement. A charge of S$50 (replacement) or S$100 (postponement) will be imposed on such changes.
Cancellation
A notice of cancellation must be given at least 3 working days before commencement of the course and 50% of course fee will be charged /returned. If notice is given less than 3 working days before the first day of the course, there will be no refund/100% charged.
